Petition for an Order of Nondisclosure- The Office of Court Administration (OCA) is statutorily directed to prescribe a petition for an order of nondisclosure. See Texas Government Code, Section 411.081(f-1). Pursuant to this directive, OCA has published related documents on their website.

Record of Support (Data Gathering Form) - Attorneys are encouraged to complete this form when submitting court ordered payments. Once received at the district clerk office, the data will be entered on TXCSES Web (Texas Child Support Enforcement System), and will create an immediate account for child support payment from the non-custodial parent. By utilizing this form, the amount of time it takes for child support payments to be sent to the custodial parents is greatly reduced

Request to Redact SSN from Court Documents - In accordance with Chapter 552.147 (d) of the Texas Government Code, this form is provided for the redaction of an individual’s social security number (all but the last four (4) digits).

Statement of Inability to Afford Payment of Court Costs - Any party declaring an inability to pay court costs must use the form Statement approved by the Supreme Court (effective Sept. 1, 2016), or the Statement must include the information required by the Court-approved form.

Application/Order to Unseal Adoption Records - Proper identification must be provided when filing this request. If request is being made by mail, please include a copy for verification.

Protective Order Registry Form & Order - Senate Bill 325, enacted by the 86th Legislature, requires the Office of Court Administration (“OCA”) to create an online registry for family violence protective orders and applications. Court clerks are to enter the documents and information into the registry, and specified officials have full access to the registry’s contents. Upon a protected person’s request and OCA approval, the public has access to more limited information. The bill requires the Court to prescribe a form for use by a protected person to request or remove public access to that more limited information.
